These terms govern your use of Body Forward websites, services, content, communications, and access to services offered by third-party medical groups, providers, and pharmacies.

If you have a medical emergency, call 911 or seek in-person emergency care immediately. This service is not appropriate for every medical condition.

Acceptance of terms

By accessing or using the service, creating an account, or affirmatively accepting these terms, you agree to be bound by them. If you do not agree, do not use the service. We may update these terms by posting a revised version; continued use after posting constitutes acceptance.

Relationship with Body Forward and clinical parties

Body Forward provides a technology and coordination platform. Body Forward is not a healthcare provider and does not control the practice of medicine. Medical groups and licensed providers are independently responsible for clinical decisions. Pharmacies are independently responsible for pharmacy services. Using the service may establish a provider-patient relationship with a medical group or provider, but not with Body Forward.

Participating medical groups and pharmacies

The legacy agreement identifies Qualiphy PC and Unparalleled Care PC as participating medical groups and Strive Compounding Pharmacy, Rush Pharmacy, StrikerRx, Hallandale Pharmacy, Empower Pharmacy, Nova Specialty Pharmacy, and Greenwich RX as pharmacy partners. Prescriptions may be transferred among participating pharmacies as permitted by law and based on treatment and location.

Eligibility and availability

Services are available only where participating providers and pharmacies are authorized to operate. Unless a specific service states otherwise, users must be at least 18 years old or the age of majority in their jurisdiction. The service is not intended for children under 13.

Prescription products

Prescription products require consultation with a licensed provider, a determination that treatment is clinically appropriate, and a valid prescription. A requested prescription is not guaranteed. Fulfillment and shipping may be handled by a partner pharmacy, and availability varies by location and treatment. Follow all provider and pharmacy instructions.

Consent to telehealth

Telehealth uses electronic communications and technology to provide healthcare when a patient and provider are in different locations. It offers potential benefits and also carries risks, and it is not a substitute for in-person care in every situation. Before receiving telehealth care, you will be required to review and accept the applicable informed consent presented through the service.

Information and technology requirements

You must provide truthful, accurate, and complete information and maintain the device, internet access, software, and connectivity needed to use the service. A provider may determine that an in-person visit or another type of care is required.

Financial responsibility

Services are generally offered on a cash-pay basis. You are responsible for charges disclosed before purchase. Unless expressly stated otherwise, Body Forward and participating medical groups do not submit claims to Medicare, Medicaid, or other government healthcare programs.

Orders, payment, and delivery

Products are subject to availability, quantity limits, price changes, and applicable taxes. You authorize charges to the payment method submitted at checkout. Shipping dates are estimates, and risk of loss passes as described in the service agreement and applicable law. Body Forward may correct pricing errors, revise an order, cancel an unavailable order, or issue an applicable refund.

Communications and account security

You are responsible for keeping account credentials secure and for reviewing service messages, reports, and treatment instructions. You consent to necessary electronic communications related to your account and requested services, subject to applicable law.

Disclaimers and limitation

Information on the website is general information and is not a substitute for professional medical advice. Results vary. To the fullest extent permitted by law, services are provided without warranties not expressly stated, and liability is limited as provided by applicable law.

Dispute resolution and arbitration

Except for limited claims described in the agreement, disputes are resolved through binding individual arbitration administered by the American Arbitration Association under the Federal Arbitration Act. The agreement waives jury trial and class or representative proceedings. Small-claims and specified intellectual-property remedies may remain available.
